What's New in Medicare for 2025?

Each year, the Centers for Medicare & Medicaid Services (CMS) announces updates to Medicare costs, coverage policies, and program rules. Staying current with these changes ensures you're not caught off guard by new costs or missed opportunities. Here's what Medicare beneficiaries need to know heading into 2025.

Part D Prescription Drug Cost Cap

One of the most significant changes taking effect in 2025 is the new $2,000 annual out-of-pocket cap on Part D prescription drug costs. This change, made possible through the Inflation Reduction Act, is a landmark shift for Medicare drug coverage:

  • For the first time, there is a hard cap on what you'll pay out of pocket for covered prescription drugs under Part D each year
  • Once you reach $2,000 in out-of-pocket drug costs, you pay nothing more for covered drugs for the rest of the year
  • This effectively eliminates the previous "catastrophic phase" cost-sharing structure
  • Beneficiaries with high-cost medications — such as those for cancer, rheumatoid arthritis, or multiple sclerosis — stand to benefit significantly

Medicare Prescription Payment Plan

Also launching in 2025, the Medicare Prescription Payment Plan allows Part D enrollees to spread their drug costs throughout the year rather than paying large amounts all at once:

  • You can opt in to pay monthly installments instead of a lump sum at the pharmacy
  • No interest is charged on the payment plan
  • This is especially helpful for beneficiaries who take expensive specialty medications at the start of the year
  • Contact your Part D plan to opt in — enrollment is voluntary

Part B Premium and Deductible Adjustments

CMS adjusts Part B costs annually. For 2025:

  • The standard Part B monthly premium has been updated — check Medicare.gov or your official Medicare notice for the current figure, as it is income-adjusted for higher earners
  • The Part B annual deductible has also been updated for 2025
  • Higher-income beneficiaries should review their IRMAA bracket, which is calculated based on income from two years prior

Part A Updates

  • The inpatient hospital deductible has been adjusted for 2025 — review your Medicare & You handbook for exact figures
  • Coinsurance rates for extended hospital stays have been updated accordingly
  • Skilled nursing facility coinsurance rates have also been adjusted

Expanded Mental Health Coverage

Medicare continues to expand access to mental health services in 2025:

  • Coverage for marriage and family therapists and mental health counselors is now available under Medicare
  • This expands the types of providers you can see for mental health treatment without needing a physician referral
  • Telehealth mental health services continue to be covered following expanded access established in recent years

Telehealth Coverage Continues

Telehealth benefits that were expanded in recent years continue to be available in 2025:

  • You can receive many Medicare-covered services via video or phone from your home
  • Telehealth access for behavioral health and primary care visits remains broadly available
  • Check with your provider or plan to confirm which services are available via telehealth in your area

Medicare Advantage Plan Landscape

The Medicare Advantage marketplace continues to evolve in 2025:

  • The number of available plans has shifted — some plans have exited markets or reduced benefits
  • CMS has increased oversight of supplemental benefit offerings to ensure they provide genuine value
  • If you're on Medicare Advantage, it's especially important to review your plan's Annual Notice of Change (ANOC) to understand what's changing for 2025
